Abstract (Document Summary)
Oil continued to pour into the river for 12 hours before the flow was killed because pipeline operators initially thought the problem was a "column separation," when bubbles in the gas condensate impede the flow of oil. Because the proper response to a column separation is to increase the pressure, the misdiagnosis was devastating, the report says.
